Please note that we do NOT accept appointment requests,  cancellations or changes through email. You must call the clinic.  

24 hour advance notice is required to cancel an appointment without penalties. Cancellations made within 24 hours of the scheduled appointment will be considered a 'no show'. 

1st no show : Patient will be notified and "no show" will be recorded in their chart

2nd no show : $40 no show fee will be charged

3rd no show : $65 no show fee will be charged 

4th no show: $100 no show fee will be charged 

* More than 4 no shows may result in termination of services
